J-1 Scholar or Student Intern Departure

For record-keeping and immigration reporting purposes, International Student & Scholar Services needs to know when the J-1 Scholar or Student Intern leaves the University of Colorado Denver | Anschutz Medical Campus or completes J-1 status.

The HR Business Partner or the contact for the host unit must complete the J-1 Departure Form within 2-3 weeks prior to the J-1 Scholar or Student Intern's departure.

After submission of the form, short surveys will be sent to faculty host and J-1 scholar or student intern about their experience during the J-1 program at the University of Colorado Denver | Anschutz Medical Campus. ISSS uses the information to evaluate the J program and to report to the U.S. Department of State as required by federal regulation.

Contact ISSS at J-1scholars@ucdenver.edu to be granted access to the ISSS case management system, MyISSS, to submit a J-1 Departure Form. If you already have access to MyISSS, submit the Departure Form in MyISSS.
