Once you have completed a separate Security Monitored Equipment Notification form for each freezer/piece of equipment to be connected, we will contact you to obtain a signature and make arrangements to evaluate the equipment for connection.
Once you’ve completed the entire form, please fax or e-mail it to 303-724-1352 or idaccess.badges@ucdenver.edu.
