Forms and Surveys
The university offers three software tools that provide the ability to create online forms and collect data: Formstack, Microsoft Forms and Qualtrics. These tools vary in levels of complexity and flexibility and are available for all faculty, staff and students based on your needs.
Please note: If you are collecting highly sensitive information such as HIPAA (Health Insurance Portability and Accountability Act) patient protected health information, you must use Qualtrics to collect this data. Formstack is not HIPAA compliant. As of January 2021, all forms being used to collect patient data must be converted to a Qualtrics form.
Formstack
Formstack is a form building and data collection tool that can be used to accomplish a wide variety of tasks. There are three main areas of Formstack:
- Form Builder - Select different types of fields and menus to design your form
- Form Settings - Cconfigure everything from notification emails to approval workflows and form security
- Submissions Database - View, edit, export or manipulate the data which has been submitted via your form
As a reminder, if you currently have patient data in Formstack, be sure you to export the data to a secure location and delete all protected data from Formstack. As of January 2021, all forms that collect protected information will be required to reside in Qualtrics.

Microsoft Forms
Microsoft Forms is a tool that allows for the easy creation of surveys, quizzes, and polls. Microsoft 365 applications are free to faculty, students and staff. Visit OIT's Microsoft 365 webpage for more information about Microsoft software or go to https://office.com to access your Microsoft 365 applications.
The university has a Business Associate Agreement (BAA) with Microsoft that allows for secure collaboration and HIPAA compliance. Visit the HIPAA compliance for Teams webpage for more information.
Get Started
- Forms can be accessed under the waffle app launcher in the top left corner of Microsoft 365.
- If you have the Microsoft Teams desktop app installed, you can also access Forms under Apps > All apps > Forms.
Qualtrics
Qualtrics provides a cloud-based survey platform allowing organizations to standardize data and surveys. All faculty, students and staff have access to Qualtrics at no cost.
The university has a Business Associate Agreement (BAA) with Qualtrics and is an approved software provider for forms that collect protected information. This includes information as part of the Health Insurance Portability and Accountability Act (HIPAA). All new forms that collect protected information are required to reside in Qualtrics as of January 2021.
