Certificate Programs

Application Deadline (summer start): February 15
The Colorado School of Public Health certificate programs offer students the opportunity to enroll in graduate public health courses to complete a standalone certificate. If you are planning to use the certificate as an entry point for the Master of Public Health, please contact your academic advisor for elective course suggestions. To apply for admission to the Colorado School of Public Health certificate programs, applicants are required to complete the Schools of Public Health Association Application Services (SOPHAS) Express application​ in addition to the requirements listed below. The application typically opens two months prior to the deadline. 


 Official Transcripts

You must have received a bachelor's degree from an accredited institution. Additionally, you must submit official copies of your transcripts from every insitution you have attended with your application. Transcripts from any non-US colleges or universities must be evaluated by World Education Services.

Official ​transcripts must be emailed ( or mailed to the Office of Admissions & Student Affairs.​


 Personal Statement

Your personal ​statement should be 2 pages in length, single spaced, and should address the following points:

  1. How will this certificate training contribute to your career plans?

  2. Describe any domestic and international work or volunteer experiences that shaped your interest in pursuing the certificate program to which you are applying​.


 Resume or CV

Please ​submit a recent copy of your resume or curriculum vitae (CV).​​​​​


 One (1) Letter of Recommendation

It is highly encouraged that your letter of recommendation come from a faculty member who can attest to your academic performance.​​​​​​​


​​​​​In addition the general admissions requirements listed above, students applying to the Colorado School of Public Health from outside of the United States must submit the following documents:

 Transcript Evaluation

All academic transcripts must be submitted to the World Education Services​ for evaluation.​​​​


 Official TOEFL Scores

ETS ID: 4875

In order to demonstrate english-language proficiency, you must submit an official copy of your TOEFL scores.


 Financial Statement (admitted students only)

​The financial statement is only for admitted applicants and is submitted at the time of admission. 

It provides the Colorado School of Public Health with information related to your ability to pay for the certificate program. Please note, the financial statement must be translated into English.


 I-20 Form (admitted students only)

The I-20 is only fo​r admitted applicants and is submitted at the time of admission.

The I-20 form gives the Colorado School of Public Health the information needed to request your student visa from the United States government.​


​​​​Application Process & Instructions
  1. Go to SOPHAS Express and create an account.


  3. Fill out the Applicant Information, Academic History, Supporting Information, ​​and Designations section. ** For Designations, if you are planning on choosing the Public Health Sciences Certificate, be sure to choose either the University of Northern Colorado campus, the University of Colorado Anschutz Medical Campus, or the Colorado State University Campus. 


  5. In the Supporting Information section, upload your Statement of Interest (see above) in the Attachments section.


  7. In the Supporting Information section, click on Evaluations to submit your request for a letter of recommendation​.  Provide the name and email address of the person completing your letter of recommendation and an email and link from SOPHAS Express will automatically be generated to that person.


  9. Have your official college transcripts sent directly from your colleges or universities (or from WES if you are an international student) to the address below. Transcripts sent by applicants will not be accepted.
