NIH “K” Awards
New and Competing

Use PHS 398 forms and Section IV instructions.

Documents required, and the order in which they should be submitted:

  1. Face Page (Form Page 1)
    • A sample Face Page with UC Denver administrative information is available on the Grants and Contracts Sponsor Forms page.
    • Leave items 8a and 8b blank
  2. Description and Personnel (Form Page 2)
  3. Table of Contents (RCA Substitute Form Page 3)
  4. Detailed Budget for Initial Budget Period (Form Page 4)
    • DO NOT submit (will be required prior to award)
  5. Budget for Entire Proposed Period of Support (Form Page 5)
    • DO NOT complete categorical budget table; complete only the requested total direct costs for each year and for the entire proposed period of support
  6. Budget Justification
    • Justify each key personnel, consultants, and any major budget items, other than personnel, that would be considered unusual for the scope of research (e.g., large pieces of equipment)
  7. Biographical Sketches (see Biographical Sketch Format Page)
    • Required for the candidate, sponsor(s), co-sponsor(s) and any Key Personnel
    • May be 4 pages in length
    • Sections A (Postions and Honors) and B (Publications) together may not exceed two of the four-page limit
    • Do not include award amounts or percent effort for projects in Section C (Research Support).
  8. Other Support (see Other Support Format Page)
    • Mentored awards only-submit modified Other Support for sponsor(s) only; DO NOT submit for all other career development awards
  9. Resources (see Resources Format Page)
  10. Letters of Reference (see RCA Reference Guidelines Format Page)
    • Required for all except K02, K05, K24 and experienced investigators submitting K07's
  11. Candidate information
    • background;
    • career goals and objectives; and
    • career development/training activities during award period
