​The REACH project supports Urban Indian Health Organizations across multiple states to implement policy, environmental, and systems approaches to address risk factors for diabetes, cardiovascular disease, and obesity.  The REACH project manages sub-recipient awards, providing specialized expertise and support to sub-recipients, and monitoring performance toward accomplishment of the REACH 2012 goals.


In September 2012, the Centers for American Indian and Alaska Native Health at the Colorado School of Public Health at CU Anschutz were awarded a cooperative agreement by the Centers for Disease Control and Prevention (CDC) with the objective of reducing chronic disease among urban American Indians and Alaska Natives.  Our project represents one of 6 CDC REACH awards funded as part of the REACH U.S. Initiative.​​
