Sheridan Health Services is a 501(c)(3) federally-qualified nurse-managed clinic that began in 1995 in with providers from the University of Colorado, College of Nursing. We offer a variety of services to encompass all of a patient's needs and strive to provide complete care to individuals with a focus on wellness and disease prevention. The School-Based Health Center provides primary care and a comprehensive range of physical, mental, and oral health services to a low-income, diverse, pediatric population.

Appointments

An appointment is recommended. However, if you are ill and need to be seen immediately, please call and we will make every effort to see you the same day. If you are 15 minutes late, you will need to reschedule your appointment; we are sorry for the inconvenience.

General Payment Information

Medical Care: We accept Medicaid, CHP+, Medicare, CICP, Doctors Care and some private insurance plans. Currently we are contracted with Cigna and Anthem. Please contact us for any updates to our list of private insurance contracts.

Dental Care: We accept Medicaid, CHP+/Colorado Access, Delta Dental, and self-pay patients. Payment for most dental care is expected the time of services rendered; however some extensive work, such as dentures and partials may require a pre-procedure deposit. Please ensure that your dental coverage is current and active and that you bring the sufficient funds for any out of pocket expenses on the day of your appointment. 

If you are uninsured, you may be eligible for our Sliding Fee Discount Program. You must set an appointment with our outreach and enrollment specialists in order to be screened for eligibility. Please call the enrollment specialists at 303-315-6162 or 303-315-6151 to set up an appointment.

We are also able to register eligible clients for Medicaid or with the Connect for Health Colorado insurance exchange.​​
