Introduction
The Seniors Clinic is now located in its new facility in the Outpatient Pavilion on the Anschutz Medical Campus. Our new home is just six miles east of our old location and you will find it is one of the most advanced health care facilities in the world. This is an exciting time for all of us at the University of Colorado Hospital. Few times in history has an entire hospital system relocated to a brand new, state-of-the-art facility. The University Hospital Seniors Clinic was first established at the University of Colorado Hospital in 1990. The Clinic seeks to promote wellness in old age, enhance functional capability, reduce medication burden, improve treatment of age-related diseases, and provide the most appropriate treatment near the end of life. The clinic has a multidisciplinary approach to the care of older adults and includes physicians, mid-level providers, nurses, pharmacists and a social worker. Each of these members of the care team brings unique knowledge and skills to the care of older adults.
The goal of the Clinic is to provide the highest quality primary care experience to our patients, their families and caregivers.
Practicum
As part of the University of Colorado School of Medicine, The Seniors Clinic currently provides an outstanding clinical training site for geriatric fellows, internal medicine residents, medical students, nurse practitioner and physician assistant students, doctor of pharmacy students, graduate level social work students, and dental students. In this multidisciplinary environment, all of these trainees learn to work together to provide the best care for complicated older patients.
Eligibility & Services
Eligibility: All persons age 75 and older with Medicare A/B. We currently participate in some (but not all) Medicare managed care contracts. Please call for specific details about which Medicare managed care contracts we accept. For further information call: (720)848-3400.
Services: The University Hospital Senior's Clinic provides the full spectrum of primary health services and also provides consultations and second opinions.
- Ongoing primary care with both MD geriatricians and mid-level practitioners
- Consultations (memory loss, depression, others)
- Social work consultations
- Social work case management
- Pharmacy consultations and medication review
- Clinic Hours: Monday - Friday 8 a.m. to 5 p.m.; with evening and weekend on-call urgent/emergent services.
Family Conferences
Family conferences are an integral part of our care delivery. They are held throughout the continuum of care and as patients needs or concerns arise. The plan of care for the patient is re-evaluated and revised according to the needs and desires of the patient (family or caregiver). These are arranged by the social worker.
In almost all circumstances, the primary relationship is between the provider and the patient, and is held strictly confidential. While involvement of family and other caregivers is often helpful and is encouraged, it requires the approval of the patient in most cases.
