Application Requirements
Applicant must have completed a Family Medicine, Internal Medicine, or an Emergency Medicine residency program prior to fellowship. Our fellowship participates with the National Resident Matching Program (NRMP) through American Medical Society for Sports Medicine (AMSSM). Please send the following items with your application:
- An AMSSM application form can be accessed at www.amssm.org)
- A current CV
- Three letters of reference
- A letter from your current residency program director indicating your anticipated completion date and standing within the residency
- A copy of your medical school Dean’s letter/Program Evaluation letter
- A copy of your medical school transcript
- Copies of your USMLE/COMLEX scores
- A copy of your current medical license
- Residents in our program must be a U.S. citizen, lawful permanent resident, refugee, asylee, or possess the appropriate documentation to allow Resident to legally train at the University of Colorado Denver School of Medicine.
- Please record "PEDIATRICS" at the top of your application if you are a Pediatric applicant
