San Luis Valley Community Health Survey

In 2009-2011, the Rocky Mountain Prevention Research Center conducted this community health survey using a community-based participatory research approach. The end result is a comprehensive, representative community health profile that can be used for setting research and intervention priorities as well as gauging health status over time.

County Maps:

See below maps used in developing the sampling frame. A sample of approximately 1500 adults were randomly selected with a stratified, multistage cluster design and asked to complete a health survey. The sampling frame was based on: 1/ county and 2/ population density.

Costilla County

Mineral County

Rio Grande County

Saguache County
