Applicants must be a PharmD graduate of an accredited college of pharmacy and must be eligible for and obtain licensure in Colorado. The application deadline is January 2, 2012. All materials must be received by that date to ensure your application is considered. Application materials include:
- Online application completed. Applications can be submitted beginning November 1, 2011.
- Letter of intent, not to exceed two pages (uploaded with online application)
- Curriculum Vitae (uploaded with online application)
- Three letters of reference. These recommendations should be completed by your reference online. Please direct your reference to the “Online Application” link on or after November 1, 2011 for instructions. We do not require the ASHP Resident Applicant Recommendation Request Form.
- Official college of pharmacy transcript. Transcripts should be mailed directly from your school and addressed to:
