How to Apply


The number of applicants accepted varies each year based upon positions available and Commission on Dental Accreditation limitations. Candidates should possess good academic credentials.  An Externship of three to five days is encouraged to allow potential applicants to interact with faculty and residents, and determine if the applicants residency goals align with those of the Program.  Additional experience before or beyond dental school (internship or residency, military service, private practice, graduate school, etc.) strengthens the applicant’s credentials. A personal site visit and interview may be required for the benefit of both the applicant and the CUSDM faculty and is scheduled on for all invited candidates in August of each year. Applicants must have a completed application submitted by July 15th of the applicant year in order to be considered for an interview.

Required application materials include:

1. Completed application form.  Download the Postgraduate Periodontics Program Application​

2. Regents Questionnaire.  Required by the University of Colorado Board of Regents.

3. Colorado Residency Form​ (Colorado residency not a requirement)

4. Application fee of $50.00  

Additional documents required:

1.  One official transcript in a sealed envelope from each college/university attended. If you have attended a non-USA college/university, it is required that all international transcripts be evaluated by an accredited foreign credentialing service. Please contact our office for a list of such services, if needed.

2.  National Board Examination scores (official report or certified copy). 

3.  Test of English as a Foreign Language (TOEFL), if international applicant. *Must meet the minimum score of 560 on the paper test, 220 on the computer-based test, or 68 on the Internet-based test. TOEFL tests taken more than two (2) years prior to the date of application will not be accepted.

4.  Graduate Record Examination (GRE) scores. *Required for all applicants who graduated from a dental school not accredited by Commission on Dental Accreditation.  Required for all applicants from schools that do not rank or provide grades.  GRE scores taken more than five (5) years before application date will not be accepted.

5.  Letters of recommendation from three (3) individuals addressed to the Director of Graduate Periodontics.

6.  A brief curriculum vitae.

7.  Applicants who are Permanent Residents of the US must supply a certified copy of both the front and backsides of their federal Green Card. All international students must provide their full legal name as it appears on immigration documents
