Two to three applicants are accepted on an annual basis to the Periodontics Program. Candidates should possess good academic credentials and have demonstrated a definite interest in periodontics. Additional experience before or beyond dental school (internship or residency, military service, private practice, graduate school, etc.) strengthens the applicant's credentials. A personal site visit and interview are required for the benefit of both the applicant and the University of Colorado School of Dental Medicine faculty. Candidates are invited to these in August. Applicants submit a completed application by July 31st of the year prior to be considered for an interview.
Application Process
Applications can be submitted at anytime throughout the year. However, the deadline to apply to the program is July 31st of each year.

Required application materials include:
- Completed application form
- Personal Essay
- Regents Questionnaire
- Colorado Residency Form (Colorado residency not a requirement)
- 2" x 2" photograph
- Application fee of $50.00
Additional documents required (must be sent directly to Pam Colosacco at the University of Colorado School of Dental Medicine from originating institution):
- Official transcripts from all colleges and universities attended
- Official GRE scores (optional but desirable)
- Dental school grades and class standing
- Official National Board Dental Examination Part I and Part II scores
- Academic Evaluation Form from Dean of Dental School
- Three (3) Letters of Recommendation
In addition, for International Students:
- TOEFL scores
- Translated transcripts and other documents
- GRE score
- Part I National Board score
