Three applicants are accepted on an annual basis to the Periodontics Program. Applications can be submitted at anytime throughout the year, however the application submission deadline is July 15th. Incomplete applications will not be considered.
Required application materials include:
1. Completed application form. Download the Postgraduate Periodontics Program Application.
2. Academic Performance Evaluation. Completed by the Office of the Dean of the Dental School you attended. Download Form Here
3. Regents Questionnaire. Required by the University of Colorado Board of Regents.
4. Colorado Residency Form (Colorado residency not a requirement)
5. Application fee of $50.00
Additional documents required:
1. One official transcript in a sealed envelope from each college/university attended. If you have attended a non-USA college/university, it is required that all international transcripts be evaluated by an accredited foreign credentialing service. Please contact our office for a list of such services, if needed.
2. National Board Examination scores (official report or certified copy).
3. Test of English as a Foreign Language (TOEFL), if international applicant. *Must meet the minimum score of 560 on the paper test, 220 on the computer-based test, or 68 on the Internet-based test. TOEFL tests taken more than two (2) years prior to the date of application will not be accepted.
4. Graduate Record Examination (GRE) scores. *Required for all applicants who graduated from a dental school not accredited by Commission on Dental Accreditation. Required for all applicants from schools that do not rank or provide grades. GRE scores taken more than five (5) years before application date will not be accepted.
5. Letters of recommendation from three (3) individuals addressed to the Director of Graduate Periodontics.
6. A brief curriculum vitae.
7. Applicants who are Permanent Residents of the US must supply a certified copy of both the front and backsides of their federal Green Card. All international students must provide their full legal name as it appears on immigration documents
