Reporting on COVID-19 symptoms, exposure and testing is essential to the health and safety of our community. The following guide can help you determine whether to self-report.
Note that if you answer 'yes' to any of the questions below, you must self-report your status, even if you are fully vaccinated and/or boosted.
*Direct or close contact is defined as less than 6 feet apart and/or more than 15 minutes of contact.
Please note that the self-reporting form is NOT the same as the vaccine verification and exemption form and should not be used to self-report your vaccination status.
After submitting your self-report - if you do not receive an auto-clearance email allowing you to be on campus - let your supervisor/professor know and stay home until contacted with further instructions. Follow the recommendations of the CU Denver Contact Tracing Team, public health agency and/or medical provider. Please do not return to on-campus activity until officially cleared to do so.
