Course Proposal and Revision Form Course Setup If your department would like to propose a new course this form must be completely filled out. CPE Course Approval Form Course Setup The continuing and professional education course information and instructor approval form is to ensure we have the correct information when schools/colleges request to add it into ISIS ISIS Access Request Access Request The access request form must be completed for each person requesting access to ISIS. It should be signed by you and your supervisor before being forwarded to the ISIS coordinator for your campus. The speedtype and HRMS number are important fields that are frequently overlooked. Forms for The Anschutz Medical Campus Form Name Category Description Course Withdrawl Form Course Withdrawl Form This form is for students who would like to withdrawal from a course. Note: Students not eligible for refund of tuition and fees after the 10th day of the semester for fall/spring and 5th day of the semester for summer. Course Proposal Form Maintain Course Inventory This form is to be used by programs to create new courses or revise existing courses.  To be used by program designees only.  Complete form electronically, have the authorized program signature authority sign electronically and forward to Anschutz Office of the Registrar program liaison.
