COF Waiver Request Form Tuition/funding forms To be used to apply for exception to the 145 College Opportunity Fund (COF) lifetime hours.
Concurrent Registration Form Registration/schedule adjustment forms

This form may be used by degree-seeking students to register through their home campus for courses taught at other University of Colorado campuses. Concurrent registration can only be conducted during the drop/add period at the host campus; registration will not be allowed before or after this drop/add period. For more information please read the concurrent registration policy and restrictions on the form. 

Enrollment Verification Form Student Records forms Complete personal information, sign and date. Also indicate which term(s) you need verified. For dates not on form, please contact records at 303-556-2389.
Interinstitutional Application Interinstitutional forms

UC Denver students who want to take a courses at Community College of Denver will need to complete the inter-institutional application form.

Interinstitutional Registration Interinstitutional forms UC Denver students need this form to register for an interinstitutional course. You must complete the application above before submitting this form.
Military Waiver for Resident Tuition Form Tuition/funding forms This form is for active duty military personnel and their dependents requesting an adjustment to in-state tuition status. For more information click here.
Name and Record Update Request Form Student records forms Use this form to let the Registrar's Office know of a name change or if your student information no your record is incorrect.
Release of Confidential Information Request Form Student records forms Authorizes UC Denver officials to release student information on transcript, grades, GPA, course schedule, financial tuition balance/payments, financial aid, COF status, birth date and student stops to the person/organization specified in the form.
Request to Prevent Disclosure of Directory Information Student records forms Privacy form allows the records office to put a privacy flag on your student records. PLEASE NOTE: This can be an inconvenience if you are seeking employment. The records office cannot release your enrollment, degree status or major to anyone unless you come to the records office with a photo ID.
Request to Receive Access to Non-Directory Information of a Dependent Student Student records forms Parents who have the student listed as a dependent on their most recent income tax return need this form to receive a copy of the dependent student's academic record.
Schedule Adjustment Form Registration/schedule adjustment forms To be used only when registration options are not available on Web registration (S.M.A.R.T) or when signatures are required for adding or dropping.
Special Processing Form Registration/schedule adjustment forms Use this form to register for independent study, thesis, practicum, special studies, variable credit, etc.
Special Tuition Rate Form Tuition/funding forms Nondegree students who have previously earned a baccalaureate degree and are taking undergraduate courses ONLY can be assessed undergraduate tuition.
Transcript Request Form Student records forms Request a transcript online at S.M.A.R.T. or complete this form to have your UC Denver transcript sent or faxed to another institution, employer, etc.
Transfer Credit Review Form Student records forms This form will initiate the faculty review of transfer course(s). The results from this review will be added to a student's transfer record on the student information system (SIS) and will be applied towards the appropriate degree requirement(s). Please contact an undergraduate advisor to discuss the use of this form in more detail.
Tuition Appeals Form Tuition/funding forms If you feel that you have an extenuating circumstance that justifies an exception to financial obligations, you may appeal and it will be reviewed by the tuition appeals coordinator.
Undergraduate Nondegree Petition Registration/schedule adjustment forms To withdraw/drop after deadline dates - This form is to be used by undergraduate nondegree seeking students only. Submit this to the Office of the Registrar for review. If you are in a degree program you must petition your dean directly.

Course Withdrawal Form Registration/schedule adjustment forms This form is for students who would like to withdraw from a course. Note: Students are not eligible for refund of tuition and fees after the 10th day of the semester for fall/spring and 5th day of the semester for summer.

House Bill 1023 Form Tuition/funding forms The following affidavit form is to signify your compliance of lawful presence within the United States by submitting proof of identification.
House Bill 1023 Form (Not physically present) Tuition/funding forms The following affidavit form is to signify your compliance of lawful presence within the United States by submitting proof of identification. This is for students who are not physically present.

Use this form to notify the Registrar's Office of a name change or your student information of record is incorrect.

Non-Degree Student Application Registration/schedule adjustment forms This form is for students who plan to take nondegree core courses for the schools and programs of medicine, dental medicine, physical therapy, public health, CHA/PA and genetic counseling, which are not open to nondegree students unless the signature of the appropriate associate dean or director of the school/program is obtained.
Petition for In-State Tuition Classification Tuition/funding forms For current and former UC Denver students to apply for a change from out-of-state residency to in-state residency for tuition purposes. Please read the information attached to the form for more information.

Rush Transcript Request Student records forms Complete this form to have your UC Denver transcript sent or faxed to another institution, employer, etc. with a rush status.
