• I have a disability that prevents me from using standard instructional materials.
• The alternative format instructional material I am requesting is in connection with a course in which I am registered for, and the textbook is required for this course.
• I have purchased the textbook that is required for this class.
• I will use the alternative format of the instructional material solely for my own educational purposes.
• I understand that UCD permits me to use these alternative format instructional materials.
• I will not copy nor distribute these alternative format instructional materials.
• I understand that alternative format materials are protected by copyright laws where no part of the materials I’m provided may be reproduced or used in any form or by any means &mdash graphic, electronic, or mechanical, including photocopying, recording, taping, web distribution, or information storage and retrieval systems &mdash without the written permission of the publisher.
