Welcome to the University of Colorado Practice Transformation Website!

It is an incredibly exciting time to be in healthcare, especially in Colorado! Numerous efforts are being spearheaded around medical homes, systems of care, transitions of care, meaningful use and be­havioral health integration. Many of these efforts, a countless number of you, of practices and healthcare organizations have been involved in for years in varying degrees. Many of you have been leaders and others are interested in becoming more involved and evolving in your efforts. The opportunity to do these things, to further collaborate and continue building Colorado’s foundation are presenting themselves. It is time to take an “all in” approach to improving the health of Coloradans, balancing a focus of reducing the cost of care, while also im­proving the experience of care for patients, families and health care teams.

This site was created to provide information for practices, practice transformation organizations and community members interested in learning more about participating in the Colorado State Innovation Model (SIM) and about practice transformation in general.

To find out more about Colorado SIM overall go to the website: ColoradoSIM. To find out more about applying to participate in the SIM Practice Transformation Program, please refer to the “For Practices” tab on this website where you can find the SIM Practice Request for Application (RFA), a link to the on-line application, and FAQ’s. Questions submitted via e-mail or voicemail will be reviewed weekly and answers posted to the FAQs document.  

In addition, under the “Resources” tab, links to the documents that are referred to in the SIM Practice RFA are provided, as well as other related information which may be of interest to both practices and practice transformation organizations. Also under the “Resources” tab are links to other websites, programs and general information titled Websites & Links that may provide additional information related to SIM efforts, as well as integrated physical and behavioral health in general.
