Electronic Submission - Step by step process video
The Exempt Electronic Submission video is also applicable for the Expedited Submission and Full Board Submission process. The only difference in the video for these other processes is to select the appropriate Eform.

However, If you have an active protocol that was previously submitted in paper and you wish to submit an amendment to these paper documents, or if you need to complete a Continuing Review form on an Expedited or Full Board protocol, Please download the following Word documents and submit in paper form.
