Completing this form will allow delegates or other research personnel listed on a study to make electronic submissions via the eRA(InfoEd) system on behalf of the PI. The PI is usually required to make all electronic submissions; however, the PI can complete this form, then delegates or other research peronnel can upload the form into the submission and submit electronically on behalf of the PI.

Please note that this completed form must be uploaded into every submission that delegates or other research personnel make on behalf of the PI.
