NIH Training Grants - New and Competing

Use PHS 398 forms and Section V instructions

Documents required, and the order in which they should be submitted:

  1. Face Page (Form Page 1)
  2. Description, Performance Sites and Key Personnel (Form Page 2)
    Should include information about the:
    • Research areas and disciplines
    • Levels of training
    • Numbers and background experience of trainees
    • Anticipated length of training program
    • Primary facilities
  3. Table of Contents (NRSA Substitute Form Page 3)
  4. Detailed Budget for Initial Budget Period (NRSA Substitute Form Page 4)
  5. Budget for Entire Proposed Period of Support (NRSA Substitute Form Page 5)
  6. Biographical Sketches (see Biographical Sketch Format Page)
    • All professional personnel contributing to the training program
    • May be 4 pages in length
    • Sections A (Postions and Honors) and B (Publications) together may not exceed two of the four-page limit
    • Do not include award amounts or percent effort for projects in Section C (Research Support).
  7. Resources (see Resources Format Page)
  8. Research Training Program Plan (use Continuation Pages as necessary)
    • sections a-d of the research plan should not exceed 25 pages
    • for specific elements of plan to include please see detailed instructions in Section V of the PHS 398 instruction book
  9. Checklist (Checklist Form Page)
  10. Appendices (maximum 10)

Non-Competing Renewal of Institutional Training Grants

Use PHS 2590 forms revised 04/06 and Section V instructions

Documents required, and the order in which they should be submitted:

Face Page (Form Page 1)

  • A sample Face Page with UC Denver administrative information is available on the Grants and Contracts Sponsor Forms page.

Next Budget Period (Kirschstein-NRSA Additional Budget Page 2)

For non-competing continuation progress reports where the last competing award was issued prior to fiscal year 2006 (October 1, 2005 through September 30, 2006), complete this form the same as for previous progress reports:

  • Stipends: Use current stipend levels regardless of committed funding amounts in previous award notice(s). Current levels are listed in NIH Notice NOT-OD-06-026.
  • Tuition, Fees and Health Insurance: Itemize tuition, individual fees, and either self-only or family medical insurance. Tuition at the postdoctoral level is limited to that required for specified courses.
  • Trainee Travel: State the purpose of any travel; give the number of trips involved, the destinations, and the number of individuals for whom funds are requested. Justify foreign travel in detail.
  • Training-Related Expenses: Request as a lump sum based on the predetermined amount per predoctoral and postdoctoral trainee. Enter the total dollar figure only. No further itemization or explanation is required.

For non-competing continuation progress reports where the last competing award was issued after September 30, 2006, complete this form using the new instructions for Tuition, Fees, Health Insurance and Training-Related Expenses:

  • Stipends: Use current stipend levels regardless of committed funding amounts in previous award notice(s). Current levels are listed in NIH Notice NOT-OD-06-026.
  • Tuition, Fees (excluding Health Insurance): Itemize tuition and individual fees only. Tuition at the postdoctoral level is limited to that required for specified courses. Only these costs are excluded from the F&A calculation.
  • Trainee Travel: State the purpose of any travel; give the number of trips involved, the destinations, and the number of individuals for whom funds are requested. Justify foreign travel in detail.
  • Training-Related Expenses (with Health Insurance) : Request as a lump sum based on the predetermined amount per predoctoral and postdoctoral trainee listed in NIH Notice NOT-OD-06-093. Enter the total dollar figure only. While Health Insurance is included as part of this category and will be awarded as a lump sum based on the new policy, the costs of applicable health insurance for potential trainees should be separately identified in the budget. Health Insurance costs are not excluded from the F&A calculation.

Budget Justification (Form Page 3)

Include the following information only. Additional justification of expenses is not required.

  • Indicate whether all stipends awarded for the current budget period will be used and explain any estimated unexpended balance.
  • Explain any rebudgeting from trainee positions (stipends) into tuition and fees that has occurred in the current budget period, including the number of trainee positions (predoctoral and postdoctoral) and the estimated dollar figure that was rebudgeted during the budget period being reported.
  • If rebudgeting is planned in the upcoming budget period, include similar information on those plans as well.

Biographical Sketches (Biographical Sketch Format Page)

  • For new training faculty only.
  • Maximum 4 pages in length.
  • Do not include award amounts or percent effort for projects in Section C (Research Support)

Other Support - Not applicable.

Progress Report Summary (Form Page 5)

A. Training Program

  • Provide a brief description of the training objectives and goals for the reporting period.
  • Describe the nature of the instruction in the responsible conduct of science and the extent of trainee and faculty participation.
  • Describe activities related to recruitment of trainees from underrepresented racial and ethnic groups.

B. Study Subjects

  • Provide data on the Inclusion Enrollment Report (MS Word or PDF) only if the trainees have participated in research involving human subjects that has not been reported within the progress report of another PHS-supported project.

C. Trainees

  • Kirschstein-NRSA Summary of Trainees Additional Form Page 5
    • List trainees who have left the program, and trainees who have completed their training during this reporting period
  • Inclusion Enrollment Report Format Page
    • Use this form to summarize information on the gender and racial/ethnic distribution of the trainees supported on this grant during the reporting period.
  • Additional Information (use Continuation Pages as needed)
    • Include a brief paragraph that describes the research project and course work of current trainees. Provide the name of the faculty supervisor.
    • List the titles and complete references (author(s), journal or book, year, page number) of all trainee publications not previously reported.
