Equipment may be checked out by School of Pharmacy
Faculty and Staff ONLY. Students who need equipment must have a faculty or staff member check the equipment out.
To request equipment, please fill out the information below and then select the "Submit" button.
Click the checkbox for all items that you wish to request. If you need multiple quantities of any item, please indicate the quantity you are requesting in the Comments section. You may also indicate multiple dates in the Comments if you need the items repeatedly.
To help ensure that equipment and staff are available to handle your request, please submit your request for equipment at least one full business day in advance of the time you wish to pick up the equipment. Equipment may be picked up from and returned to Room 1106 in the Pharmacy Building.
