Sheridan Health Services is a nurse-managed community health care center providing medical, dental and behavioral healthcare to the Sheridan community and surrounding areas.
The pediatric site at Sheridan Health Services has been operating out of the Sheridan Middle School location for 17 years. The goal of the clinic is to provide a location for comprehensive pediatric care for children with Medicaid, CHP+/Colorado Access, United (in process), Anthem (in process), or self-pay. The clinic is able to offer same day sick visits for patients needing to be seen immediately. Well child visits are done for all children on a yearly basis and as needed for sports physicals.
The Sheridan Health Services community clinic opened in March 2011 and has since been providing care for adults aged 18+. We accept Medicaid, Medicare, CHP+/Colorado Access, United (in process), Anthem (in process), and self-pay patients.
Nurse midwives at Sheridan Health Services offer complete care from the conception through delivery.
Sheridan Health Services employs two RN case managers on site to help to obtain low cost follow-up care for our patients and families. Chronic conditions often require services that are not provided in our clinic, but it is the role of these case managers to find the appropriate specialty service provider that will see our patients at an affordable rate.
Sheridan Health Services provides comprehensive dental care for the community. We accept Medicaid, CHP+/Colorado Access, Delta Dental, and self-pay patients.
Sheridan Health Services provides behavioral and mental health counseling services to children, adolescents and adults. We focus on promoting health by providing individualized, solution-focused treatment services with a strengths based approach. Therapeutic services include assessment & evaluation; individual therapy for ages four through adult; family therapy; substance abuse services; medication evaluation and management and referrals. Services are based on an integrated care model centered in physical/behavioral health and well-being. School based services are provided in all Sheridan schools.
