Sheridan Health Services employs two RN case managers on site to help to obtain low cost follow-up care for our patients and families. Chronic conditions often require services that are not provided in our clinic, but it is the role of these case managers to find the appropriate specialty service provider that will see our patients at an affordable rate. Common referrals followed by our case managers include:
- Reduced cost medication assistance programs
- MRI/X-ray
- Skin evaluations
- Breast cancer screening
- Cancer Care
- Smoking assistance programs
