Who We Are & What We Do

Sheridan Health Services is a 501(c)(3) nurse-managed clinic, centered on a patient-centered medical home philosophy. Providing a variety of services to encompass all of a patient's needs, we strive to provide complete care to individuals, with a focus on wellness and disease prevention.

Sheridan Health Services has two clinic locations: 1) a school-based clinic on the south side of Sheridan Middle School and, 2) a community clinic just up the street on the Fort Logan campus. A variety of healthcare services are offered at both locations. We accept Medicaid, CHP+/Colorado Access, Medicare, United (in process), Anthem (in process), and self-pay.


An appointment is recommended. However, if you are ill and need to be seen immediately, please call and we will make every effort to see you the same day.

If you are 15 MINS LATE, you will need to RESCHEDULE your appointment, we are sorry for the inconvenience.

We do NOT manage chronic pain with narcotics at this clinic.

General Pricing:

Per changes authorized by the advisory board, self pay prices will be as follows:

  • New Patient Sick Visit = $80
  • Return Patient Sick Visit/Follow Up = $40
  • Physicals = $80

We accept cash and checks. Sorry, we do not accept credit cards. Please make all checks payable to The Regents of the University of Colorado. Please call the clinic with more prices on other labs and tests.

We accept Medicaid, Colorado Access, CHP+, Medicare and some private insurance plans.

If you are uninsured, we use a sliding scale for fees based on your gross family income (this fee is for the cost of the visit only and does apply to lab costs). We are also able to register eligible pregnant women and children for Medicaid.
