Care Coordination Community of Practice

The group coordinating this work is an Ad hoc cross-agency committee with representatives from the Colorado Department of Public Health and Environment, Family Voices Colorado, and JFK Partners/University of Colorado School of Medicine.

This work is intended to have relevance to agencies, providers, and families across various disciplines and across Colorado.  Though this work has been developed with a focus on children and youth under age 22 and their families, we envision these materials can be used as a template for care coordination that spans a person’s lifetime.  While some of the language used in this document may be different than the language used in certain discipline areas, the intent is to customize the language to align it with work happening across systems as this information is shared with a variety of stakeholders.

Background
In 2008, Colorado’s early childhood mental health system-of-care initiative, Project BLOOM, sponsored four monthly forums in partnership with Colorado’s Medical Home Initiative to promote dialogue on the integration of children’s primary care and behavioral health care, and identify and discuss practical strategies for integrating family-centered primary care and behavioral health care at the practice, program, system and private sector levels. The result of the forums was the convening of the Colorado Care Coordination Community of Practice, and the development of a care coordination plan, toolkit and a document defining care coordination and its functions and expected outcomes.  

"Quality problems and spiraling costs have resulted in widespread interest in solutions that improve the effectiveness and efficiency of the health care system. Care coordination has been identified by the Institute of Medicine as one of the key strategies for potentially accomplishing these improvements” (McDonald KM, Sundaram V, Bravata DM, Lewis R, Lin N, et al., June 2007).

To offer a definition, values, functions and outcomes of care coordination to be accepted across all sources of such service, and provide care coordination guidance and resources for those involved with care coordination throughout Colorado serving children, youth and their families.



  1. To develop a definition of care coordination, and to identify the functions and outcomes of care coordination; 
  2. To offer guidance and a framework for Colorado regarding care coordination through the Care Coordination Plan; and
  3. To be a resource to collaborative conversations among entities providing care coordination.
