CU GME Vision Plan

EyeMed Vision Care 

                          Optional vision care plan offered to CU GME Residents and Fellows

                                                  Resident only                  $10.80

                                                  Resident + 1 dependent   $20.48 

                                                  Resident + family            $30.08      

                                

Vision care services, such as routine eye examinations, prescription glasses and contact lenses are not covered by the CU GME Health/Dental Benefits Plan. Residents and fellows may purchase this optional vision care plan through payroll deduction.

Enrollment

Once you enroll in the EyeMed Vision Care Plan, you cannot cancel your coverage until the next open enrollment period for coverage end date of July 31.

  • QUALIFYING EVENT ENROLLMENT: Outside of the open enrollment period (April/May), enrollment in EyeMed is allowed only within 31 days of a qualifying event (e.g. marriage or new hire).
  • NEW EyeMed Members enrolling during open enrollment (April/May): Coverage begins August 1.
  • CURRENT EyeMed Members:  It is not necessary to re-enroll during open enrollment for the next EyeMed Plan year.  Your enrollment with EyeMed will carry forward.  If you choose to cancel your coverage for the coming year (coverage end date July 31), you must cancel during open enrollment or wait until the next open enrollment period to cancel. 
  • Late enrollment (or cancellation) will not be accepted.
