You should have your own copy of the records that you provided when you entered a GME training program and also have kept a copy of PPD documentation each year when you provided it to GME’s designated provider.
If you did not retain a copy of your documentation, you may email PROHealth@dhha.org to request a copy of the information. Phone requests are not permissible. Email will serve as your consent for release of records. Records will be provided only to you (no third parties) via return email. You should keep a copy of these records and can then provide directly to requesting entities.
You may find a copy of your pay advice by going to My CU Portal. Log in instructions for this site are as follows:

Once logged in, click on My.Pay.
Go to the Payroll and Benefits Services - Forms webpage and complete the following forms:
- Address Change for Former Employees
- W2 Reissue Request Form
Fax the forms to 303-860-4299.
Salaries for each PGY level are posted on the Stipends page of the GME website and are posted for the next year once they are established.
Medical residents are not entitled to the student exception for FICA, according to the IRS Office of Chief Counsel (Tax Exempt and Governmental Entities). The IRS has concluded that generally, unless the facts are virtually identical to those in the Eighth Circuit case (University of Minnesota where residents pay tuition), services provided by medical residents are not incident to and for the purpose of pursuing a course of study, but rather a form of on-the-job training, and therefore not entitled to the statutory exception from FICA.
Print a W-4 form from the PBS Website. Complete the form and submit directly to:

Payroll & Benefit Services does NOT accept a faxed copy of the W4. The original W-4 is required. Contact Payroll & Benefits Services at 303-860-4200 with questions regarding your W-4 form. (New residents should return the initial W-4 to their program coordinator with new resident packet.)
Print a Direct Deposit Application form from the PBS Website. Complete the form and attach account documentation. The documentation may be a voided check (checking account only) or a typed letter from your bank (on bank letterhead) that lists your name, your bank’s routing number, your account number, and the type of account. Submit all information to:

The Pager Desk is located in Building 500, 4th Floor, Room C4004, next to the central IT Services department.

Replacements for lost, stolen or broken stampers will be at the Resident's expense. Approximate charge will be $11. Replacement stamper is obtained by contacting: Chris Ades, Staples, 303-373-8248 with the following information:
1. Identify yourself as a Resident with the University of Colorado Denver School of Medicine.
2. You are requesting a 2000 plus pocket stamp.
3. Provide your first and last name.
4. Mailing address where stamper can be sent.
5. VISA/Mastercard number.

All residents in GMEC approved programs are required to successfully complete the USMLE Step 3 examination or COMLEX Part 3 examination, as evidenced by obtaining a passing grade for that examination, prior to the mid-point of the second post-graduate year (PGY2).
All fellows entering GMEC approved programs must have successfully completed the USMLE Step 3 examination or COMLEX Part 3, as evidenced by obtaining a passing grade for that examination, prior to starting a fellowship.
