Prostate Cancer Surgery

Surgery is considered by many to be the "gold standard" therapy for prostate cancer. It is most commonly recommended for young patients and those with aggressive pathology. Surgery cures many symptoms from obstruction of the prostate as well as the prostate cancer itself, in most cases. The term "radical prostatectomy" describes the removal of the prostate surgically. It is only considered "radical" because the entire prostate is removed, rather than just a part. Many times, lymph nodes are taken around the prostate at the same time the prostate is removed to determine if the cancer has spread. The surgery can be performed through a four inch vertical incision under the bellybutton or by passing thin instruments and a telescope through several small incisions in the pelvis (robotic or DaVinci prostatectomy).

 Open Radical Prostatectomy & Pelvic Lymph Node Dissection

A four inch incision is made over the low part of the abdomen. Prostate cancer sometimes spreads to small glands in the pelvis called lymph nodes. These lymph nodes can be removed during the surgery to test for cancer. The prostate is detached from the bladder and a small portion of the urethra is removed. The bladder is then reconnected to the urethra. A rubber tube called a catheter is left in the penis to let the urethra heal. It is removed after 1 or 2 weeks. 1
