Verification Requests


Standard verifications from the Department of Radiology are $30 per training program and will include the trainee's: dates of training, training program, and an evaluation from the current program director.  Additional requests such as any documents associated with procedure logs, authorized user status information, mammography, etc. will be an additional fee of $30.

 Due to the current circumstances surrounding the COVID-19 virus, our Education Administration Staff are currently working from home offices.  As such, we’re still able to return trainee verifications in a professional format with department letterhead positioned at the top of the page.  And, all relevant verification information will be provided in the content of the letter. However, at this time we’re unable to provide an official program representative signature at the bottom of the page. If you must have an official program representative signature on your form, please send a verification form set up to accept our digital signatures within the context of your verification request form.   Also, at this the present time, we’re unable to obtain a University seal on any verifications until further notice.  Thank you for your cooperation during this unexpected situation.

Once the verification request and payment have been received, the verified documents will be processed within 5 to 10 business days.
