Twelve Month Curriculum


The twelve month program is designed to expose the candidate to a variety of clinical cases, to familiarize the candidate with TEE, and to help with preparation for the Oral Boards and to gain exposure to academic medicine by publishing at least one manuscript. Below is a sample schedule of a 12 month curriculum.

July:
Clinical rotation at St. Joseph Hospital in Denver. Fellow is working one on one with attending cardiac anesthesiologist and is expected to be primary care provider for duration of case. TEE requirements: Physics and Basic Knobology.

 

Academic requirements:

  • Attendance of 4 lectures:
    1. Embryological development of cardiac structures.
    2. Anatomy and Physiology of the human heart.
    3. TEE: Physical principles of Ultrasound I.
    4. TEE: Physical principles of Ultrasound II.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation
  • Take one academic day per week to work on clinical study/publish a paper.

August:
Clinical rotation at UCH-Cardiac Anesthesia.
Fellow will either be working directly with attending or supervising senior level residents on Cardio-thoracic rotation. Fellow is expected be present for case duration and along with attending anesthesiologist be responsible for all important case management decision. TEE requirements: A complete normal exam and evaluation of the left and right ventricles.

Academic requirements:

  • Attendance of 4 lectures:
    1. Anesthetic management of patients with ischemic heart disease.
    2. Anesthetic management of patients with cardiomyopathies.
    3. Evaluation of systolic function.
    4. Evaluation of diastolic dysfunction.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation
  • Take one academic day per week to work on clinical study/publish a paper.  

September:
Clinical rotation UCH-Cardiology
Fellow spends four weeks with cardiology service in order to familiarize with transthoracic echocardiography. Fellow will have ample opportunity to perform TTE as well as spend time reading transthoracic studies with attending cardiologist. Cardiology transesophageal echocardiography service will also be manned by fellow. TEE requirements: Evaluation of the atria, pulmonary veins, SVC and IVC views and appreciation of TEE artifacts.

Academic requirements:

  • Attendance of 5 lectures:
    1. Anesthetic management of patients with heart failure.
    2. Anesthetic management of patients with pericardial disease.
    3. Pericardial tamponade and constrictive pericarditis.
    4. Evaluation of RV function.
    5. Anesthetic management of patients with mitral stenosis.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation.
  • Take one academic day per week to work on clinical study/publish a paper.

October:
Clinical rotation at UCH-Cardiac Anesthesia.
Fellow will either be working directly with attending or supervising senior level residents on Cardio-thoracic rotation. Fellow is expected be present for case duration and along with attending anesthesiologist, be responsible for all important case management decisions. TEE requirements: Evaluation of the mitral valve and tricuspid valve.

Academic requirements:

  • Attendance of 5 lectures:
    1. TEE lecture: Mitral stenosis.
    2. Anesthetic management of patients with mitral regurgitation.
    3. TEE lecture: Mitral regurgitation.
    4. Anesthetic management of patients with aortic stenosis.
    5. TEE lecture: Aortic stenosis.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation.
  • Take one academic day per week to work on clinical study/publish a paper.

November:
Clinical rotation at UCH Thoracic Anesthesia.
Fellow will either be working directly with attending or supervising senior level residents on cardio-thoracic rotation. Fellow is expected be present for case duration and along with attending anesthesiologist, be responsible for all important case management decisions. Focus of this month will be the case management of patients presenting for thoracic surgery including Vats, Open, Tracheal Resection, and Tracheoplasty procedures. TEE requirements: Evaluation of the aortic valve and pulmonary valve.

Academic requirements:  

  • Attendance of 4 lectures:
    1. Anesthetic management of patients with aortic regurgitation.
    2. TEE lecture: Aortic regurgitation.
    3. TEE lecture: Evaluation of prosthetic valves.
    4. Cardiac anesthesia: Mechanical circulatory support
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock orals.
  • Take one academic day per week to work on clinical study/publish a paper.  

December:
Clinical rotation at UCH-Cardiopulmonary Bypass.
Fellow will work directly with one of our Cardiopulmonary Bypass Perfusionists. 
Fellow be exposed to the basic workings of the bypass machine as well as basic perfusion management. Fellow supervises residents during clinical cases. TEE requirements: Evaluation of the aorta and other miscellaneous TEE findings.

Academic requirements:

  • Attendance of 4 lectures:
    1. TEE lecture: Congenital heart disease I. 
    2. Cardiac anesthesia: M and M.
    3. TEE lecture: Intracardiac masses.
    4. Anesthetic management of patients with cardiac arrhythmias.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation.
  • Take one academic day per week to work on clinical study/publish a paper.

January:
Clinical rotation at Colorado Children's Hospital
Fellow will work directly Pediatric Cardiothoracic Anesthesiologists in order to learn basic management of the pediatric patient presenting with congenital heart disease. CHC is a full spectrum center of excellence in the care of congenital heart disease. Fellow will be exposed to wide array of both open and catheter based procedures.

Academic requirements:

  • Attendance of 4 lectures:
    1. TEE lecture: Congenital heart disease II. 
    2. Cardiac anesthesia: Neoplastic and infections heart diseases.
    3. TEE lecture: Pitfalls and artifacts.
    4. Cardiac anesthesia M and M.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation.
  • Take one academic day per week to work on clinical study/publish a paper.

February:
Clinical rotation at UCH-Cardiothoracic ICU
Fellow will be integral team member on CTICU service. Will work directly with CTICU trained attendings in providing management to post-surgical cardiac, thoracic, thoracic, and major vascular patients.

Academic requirements:

  • Attendance of 4 lectures:
    1. TEE lecture: 3D TEE
    2. Cardiac anesthesia: Inotropes, Vasopressors
    3. TEE lecture: Epi-aortic echocardiography
    4. Cardiac anesthesia M and M.
  • Supervision of cardiac anesthesia journal club.
  • Take two hours of mock oral board preparation.
  • Take one academic day per week to work on clinical study/publish a paper.

March-June:
Clinical rotation at UCH/CHC
Fellow will complete a total of 4-5 months of UCH-Cardiac Surgery as listed above. As well fellows will have the opportunity to take elective month/s for further experience in the field/rotation of their choosing. Previous fellows have spent more time either subspecializing in pediatric congenital cardiac, thoracic anesthesia, or in-depth clinical research endeavors.

Fellows will be evaluated monthly by the cardiothoracic faculty and quarterly by the Chairman of the Department of Anesthesiology.
