The referral and screening work of the program is carried out at the local community clinic and screening provider level. Having an uninsured population focus, the program partners with community clinics who mainly serve an underserved population.
Clinics are located all across the state and are generally part of one of the following clinic groups: ClinicNet, Colorado Community Health Network (CCHN), and Colorado Rural Health.
