Return the letter of intent and a nonrefundable tuition deposit of $4,000 to reserve your position in the class. The deposit will be applied toward tuition during the first semester of attendance. Financial aid does not cover this deposit.3
All accepted students are required to complete a background investigation conducted by Certiphi Medicine prior to matriculation. Previously obtained background checks through third parties will not be accepted. The information obtained through a background check will not become part of the student’s academic file, and will remain confidential unless the findings result in an institutional action by the School of Dental Medicine.
The final decision regarding matriculation of an applicant will be based on the self-reported information in the candidate’s application and information in the background check. Factors involved in the final decision may include, but are not limited to:
The nature, circumstances and frequency of any reported offense(s)
The length of time since the offense(s)
Available information that addresses efforts at rehabilitation
The accuracy of the information provided by the applicant
This form must be completed and signed by a health care provider or official. You must submit the original form. All immunizations (including pending immunizations in a series) must be current prior to matriculation and include measles, mumps, rubella, hepatitis B, tetanus/diphtheria,/pertussis, chicken pox, flu, and must have immunity and/or be clear of tuberculosis. There are no exceptions for any student enrolled at the Anschutz Medical Campus.
Each student is required to review, sign and submit the Technical Standards Form which defines the nonacademic criteria for admissions, advancement through and graduation from the School of Dental Medicine. 6
If you need to obtain a F1 Visa from the University of Colorado, you will need to provide that information including number of dependents and financial certification documents to the ISP office as soon as possible. Students are not required to obtain a F1 Visa to attend CU, but thier visa status must legally permit them to study in the United States. The CU School of Dental Medicine reserves the right to offer and rescind admission to applicants based on the student's visa status. Should a visa change process take an inordinate amount of time, the University may provide a deadline by which a visa change must be completed. Please discuss your visa status with the admissions office. 7
Scrubs fitting the description below must be purchased. Students will want at least 2 pair of scrubs in January to wear during clinical and pre-clinical sessions:
- Quantity: Two
- Color: Navy Blue – solid only. No prints, stripes, logos or snaps.
- Top: V-Neck; 1 or 2 pockets allowed
- Pants: Drawstring or elastic (Aviator/Cargo pocket scrubs are not allowed)
