Coordinated Health Assessment and Management Program (CHAMP)


Improving Healthcare and health coverage by supporting communities to prevent disease and improve population health

CHAMP is a collaboration between community, healthcare, K-12 and university partners to improve the health status of children in the San Luis Valley (SLV). CHAMP strengthens the community child health system through comprehensive assessments, early intervention, and team-based care linking families with health care resources.
The program provides:

  • Annual well child health assessments for K-12 school children: Hearing, Vision, Immunizations, Blood Pressure, Lung Health/Asthma, Height/Weight/BMI, Oral Health, Emotional Wellness
  • Early identification of health concerns
  • Early intervention by the school nurse
  • Assistance to families obtaining health insurance
  • Assistance to families finding a healthcare provider
  • An electronic system for collecting and sharing health information
  • Data driven program planning that addresses community needs for prevention and health promotion
  • Increased care coordination and referral completion
  • Reduced emergency and urgent care visits
  • Reduced school absenteeism for health reasons
