1. Graduate Record Examination (GRE) Test Scores

The Graduate Record Examination (GRE), or its equivalent, is required for all applicants. The University of Colorado Denver-HSC Campus code is 4875. Applicants should check with individual programs regarding the GRE and any recommended subject tests. Computer-based testing is offered year-round in many locations. Information regarding the GRE can be obtained by contacting the ETS/GRE program directly at (609) 771-7670. You can e-mail ETS staff at or the ETS Website.

2. Transcripts

Instructions for Submitting Domestic Transcripts

Two official transcripts from all university/college-level work are required. Official transcripts must be in sealed envelopes and sent by the issuing institution directly to the program to which you are applying. To locate a program address, see the “PhD Program Directory” or the “MS Program Directory”.

If you have attended the University of Colorado, you must request an official transcript. We cannot obtain one on your behalf.

Instructions for Submitting International Transcripts/Evaluations

Official copies of all transcripts in English and in the original language are required. Official translated transcripts must be sent from the institution of higher education directly to the program to which you are applying. Please see the PhD Program Directory or the MS Program Directory for program addresses. International translated transcript(s) are evaluated by the University of Colorado Denver’s International Student and Scholar Services Office.

3. Recommendation Letters

Three or four letters of recommendation are required, depending on the program to which you are applying. The letters of recommendation will be completed by your recommenders on-line. The Recommenders will receive an automatic email with instructions on how to provide the recommendations once you have submitted your  application. 

4. Graduate School Application Fee

NOTE: Transcripts should not be sent to the Graduate School address below. Please send your official transcripts directly to your program of interest. To locate a program address, see the“PhD Program Directory” or the “MS Program Directory”.

A $50.00 domestic and $75.00 international non-refundable application fee [credit card (on-line only), check, or money order] is required. No application will be processed unless this fee is paid.

The application fee is not refundable under any circumstances. If you are unable to access the internet or have difficulty with the on-line process, please email us at the Graduate School You can also contact us at the address or phone number below.
