 Disappearing bylines (headlines) in forms

Brief Description

Bylines (headlines) disappeared on all forms in both edit and published views.

Detailed Description

​On existing form subsites and new web form subsites, the headline filed (named: byline) is now missing. Any content in this field is no longer displaying on existing forms.

Solution: Roll back forms input page to original version from July 2011. This has already been dome on development. We need a quick test plan, and we should be good.

McCallister, CaseyNo presence information (2/22/2012 5:14 PM): Scott rolled back the forms layout on staging because Graham had the layout checked out on development. Once rolled back, I tested the below sites on staging and the byline appears correctly.
McCallister, CaseyNo presence information (2/21/2012 4:46 PM): The below themes and sites are the forms that I could find on my own. As said in the task, I hear that Komal has a subjective list. I've sent her an email and payed a visit to her cube, but she appears to be out for today. I feel like I'm missing a few themes, but all of the below perform correctly. 1. Identify a form site for each theme (Komal has a list of themes and representative page) on development, and test each themed form page (submission and confirmation).
2. Test to make sure there is no visual or functional weirdenss.
3. Record in spreadhseet and attach to this ticket.
